Notification of Intent to Ban Firearms from Church Premises

Instructions for Church Notification:

A person with authority to act for the church must submit a letter to the Utah Bureau of Criminal Identification. The letter must be on organization letterhead and signed by the proper church authority.

Letter must include:

Full church name
Address
Phone Number
Request to be added to the BCI Concealed Firearms website
